Responsibilities include:
Managing chronic and acute medical conditions Performing comprehensive patient assessments and physical examinations Reviewing and updating medical histories and treatment plans Collaborating with interdisciplinary healthcare teams Identifying patient care needs and recommending appropriate treatment strategies Accurately documenting patient encounters and care plans in EMR systems Promoting quality outcomes and continuity of care for senior patients Qualifications Current, unrestricted Nurse Practitioner license in state of practice Current DEA or ability to obtain DEA licensure Strong clinical assessment and communication skills Passion for geriatric and long-term care medicine Commitment to delivering compassionate, high-quality patient care Our Commitment to Inclusion PCMA is proud to be an Equal Opportunity Employer.
