Job Summary Join a dynamic and compassionate team as a Geriatric Pain & Palliative Care NP or PA , caring for residents across Cincinnati-area skilled nursing facilities as one of the first providers in the practice's new Ohio market.
Pay is productivity-based:
$10 per completed visit, with a guaranteed $150 minimum per facility seen each day. Visits are focused, ~10-minute pain management follow-ups, and providers typically see 40-50 patients daily, working out to roughly $400-$500/day. Malpractice insurance with tail coverage is included. The practice wants clinicians who've handled this pace before, with backgrounds in pain management, orthopedics, physiatry, SNF, or hospice; motivated new grads with RN experience in ortho, pain management, SNF, or a high-acuity ER (county/trauma) are also considered. Your Responsibilities Conduct focused pain assessments and design individualized treatment plans (medication management, non-pharmacological approaches, interventional referrals) Prescribe and monitor pain medications, including controlled substances, per Ohio state and federal regulations Coordinate referrals for physical therapy, interventional options, and other specialty care Manage a 40-50 patient/day caseload, refining plans through consistent follow-up Document care accurately in TEBRA and PointClickCare What We're Looking For Active, unrestricted Ohio NP or PA license required, or eligibility to obtain, with AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification Comfortable managing a high-volume caseload of 40-50 focused, ~10-minute visits/day Active DEA registration (or readiness to obtain one) required — PAs, confirm current DEA status upfront Fully onsite, comfortable traveling between Cincinnati-area facilities Priority backgrounds: pain management, ortho, physiatry, SNF rounding, or hospice with controlled-substance prescribing New grads welcome with RN background in ortho, pain management, SNF, or high-volume/trauma ER; 5+ years relevant RN experience also counts
