COMPENSATION

Salary $140k + $10k Bonus Our client is a physician-led, mission-driven primary care organization dedicated to transforming healthcare for seniors through high-quality, value-based care. Compensation:

$140k

Bonus:

Up to 10k year 1 and goes up as a partner.

• 18-20 patients a day

Hours:

Monday Friday work schedule; Clinic Hours 7:30a 5:00p (last patient is scheduled at 4:15)

• 100% outpatient
• Dedicated Care and support team (have your own MA, Scheduler & Referal Speecialist

Seniors with 3-5 chronic diseases

Door Transportation Requirements:

• Board Certified Nurse Practitioner
• 2 + years NP outpatient primary care experience
