Job Description

W2 position! We are seeking an experienced APRN in pulmonary and acute care to provide high-quality, patient-centered care in skilled nursing and long-term rehabilitation settings. This in-person role focuses on managing complex respiratory and acute care patients, including higher-acuity cases, with expected availability of 5 days per week, weekends availability and occasional after-hours on-call coverage. Dublin, OH Columbus, OH Provide pulmonary and acute care services in skilled nursing, rehabilitation, long-term care, and post-acute settings Manage mechanically ventilated patients, including ventilator settings, alarms, modes, and patient-ventilator synchrony Complete admission evaluations for new pulmonary and acute care patients Manage discharge evaluations, including respiratory stability assessments and care coordination Perform comprehensive respiratory assessments and physical examinations Evaluate and manage acute and chronic pulmonary conditions such as COPD, asthma, pneumonia, and respiratory failure Manage CPAP and BiPAP therapies, including initiation, adjustment, and monitoring Apply knowledge of ventilator mechanics and parameters (PEEP, FiO₂, tidal volume, pressures, etc.) collaboratively with physicians and respiratory therapists Manage tracheostomy patients, including trach care, assessments, and care coordination Collaborate on ventilator and tracheostomy weaning plans Interpret respiratory diagnostics, including ABGs, imaging, labs, pulse oximetry, and ventilator data Monitor response to respiratory therapies and intervene promptly to prevent decompensation Coordinate care with pulmonologists, respiratory therapists, nursing staff, attending physicians, and facility leadership Identify and manage acute changes in condition early to reduce Return-to-Hospital (RTH) events Participate in pulmonary rounds and interdisciplinary care planning Complete timely and accurate EMR documentation Educate patients, families, and staff on respiratory therapies and escalation protocols Support initiatives focused on reducing respiratory-related readmissions and emergency transfers Maintain compliance with CMS, state regulations, HIPAA, infection control, and facility policies
Required Qualifications:
Active Ohio APRN license Active NPI number Minimum 1 year of mechanical ventilator management experience Minimum 3 years of nursing experience (SNF, tracheostomy, LTAC, or post-acute preferred) Current BLS and ACLS certification Strong clinical assessment and independent decision-making skills EMR/EHR experience (PointClickCare and/or Matrix preferred) Knowledge of post-acute care and geriatric best practices Excellent communication and documentation skills Compassionate, patient-centered approach to care DEA registration preferred
