Schedule & Flexibility Shifts available any day between Monday and Sunday Set your own schedule according to your availability Commit to a minimum of hours weekly or bi-weekly based on your preferences Key Responsibilities Complete comprehensive annual risk assessments in members homes Gather and review detailed medical and medication histories Educate patients on chronic conditions, general health, and medication management Detect gaps in care and deliver actionable recommendations Focus solely on assessmentno prescribing or care plan creation required What Were Seeking Active Nurse Practitioner license in Ohio Current CPR (BLS) certification National Board Certification (AANP or ANCC) MSN from an accredited nursing program At least 6 months recent nurse practitioner experience preferred This role offers favorable compensation: youll earn $80$90 per completed evaluation, supplemented by a mileage stipend and compensation for administrative time.
