Key Responsibilities:

Perform annual health risk assessments in-home for Medicare Advantage members Review and document prior and current medical and medication histories Evaluate medical conditions through comprehensive patient interviews and observations Provide patient education regarding medical conditions and medication management Identify gaps in care and suggest actionable recommendations (no treatment planning or medication prescribing required)

Candidate Qualifications:

Active Nurse Practitioner license in the state of Ohio Current CPR certification (BLS) Board Certification via AANP or ANCC At least 6 months of recent Nurse Practitioner experience is preferred MSN degree from an accredited nursing program This per diem role offers competitive compensation of $80$90 per completed evaluation, along with mileage reimbursement and paid administrative time. Assignments span Jackson Countyincluding Jackson, Wellston, and Oak Hilland each assessment averages about 45 minutes.
