Job Description

Position Summary:
Manages and coordinates all aspects of patient care in accordance with State and Federal rules and regulations and the nursing standards based on the scope of a licensed independent practitioner.
Job Responsibilities:
Manages a clients overall care; diagnoses health problems; prescribing medications and treatments within the scope and formulary set forth by collaborating physician; treat chronic and acute conditions including injuries, infections, diabetes and high blood pressure; detect changes in patient condition and recommend further treatment options or future appointments if needed; provide after-care / ongoing care information and resources to patientsPerform initial patient screenings, patient history and physical examinations;facilitate new patient consultation; perform invasive procedures; ensures patient records are accurate at each patient appointment; administer medications; operate medical equipmentOrder, conduct, analyze and interpret diagnostic test such as x-rays, EKGs, and lab workDeliver quality, physician-level care while complying with agency policies, safety regulations, and productivity metricsEnsure patient records are in compliance and up to date with accurate and current test results, immunizations, condition changesCollaborate and consult with other healthcare professionals and multidisciplinary team members as needed;provide referrals to other practitioners when necessaryServe as a consultant and resource for patients need for ongoing care information; provide counseling and guidance for any patients with critical care conditions; facilities staff, patient and family decision making by providing educational tools; acts as patients advocate for families, individuals, groups and communities; educate patients about preventive care and prescribed treatments Facilitates the patients transition within and between health care settings (admitting,transferring and discharging patients)Provide oversight, delegation and direction to RN,LPN, and MA; ensure use of established procedures, policies,guidelines and standards under the disciplines scope of practiceOther duties as assigned
Required Knowledge, Skills and Abilities:
Excellent written and verbal communication skillsStrong customer service skillsBasic computer skills;basic mathematical skillsAdhere to acceptable professional/clinical boundaries and confidentiality Ability to work cooperatively with multidisciplinary teamsStrong medical aptitudeProficient in medical instruments and equipmentFamiliarity with the EHR medical charting systemAbility to multi-taskAbility to manage time effectively and efficientlyHigh attention to detail and thoroughnessAble to work proactively independently and as a teamHigh levelof compassion
Job Requirements:
Masters Degree in NursingPrescriptive authorization Minimum 5-years nursing experienceActive Nurse Practitioner license in OhioValid drivers license and current auto insurance
Essential Functions/Physical Demands:
Positional:
Driving in accordance within job duties assigned. Frequent standing and walking. Infrequent sitting.
Gross Mobility:
Rare lifting, climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching, bending, and handling.
Sensory:
Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
