Job Description

Advance your career as a Nurse Practitioner conducting home health risk assessments in Pike County, OH. This per diem position is designed for experienced Nurse Practitioners who value flexibility and autonomy, offering the opportunity to make a significant impact on the well-being of Medicare Advantage members residing in Waverly, Pee Pee Township, Seal Township, and surrounding areas.
Schedule:
Flexible shifts available Monday through Sunday Design your own work schedule to fit your lifestyle Minimum weekly or bi-weekly hourly commitment based on your preferences
Key Responsibilities:
Conduct comprehensive annual health risk assessments for Medicare Advantage plan members in their homes Assess prior and current medical history as well as medication regimens Deliver education to patients about their health conditions and prescribed medications Identify gaps in care and recommend actionable solutions to ensure optimal outcomes No requirement to prescribe medications or develop long-term treatment plans
Qualifications:
Active Nurse Practitioner License in the state of Ohio Board Certification through AANP or ANCC Current CPR certification (BLS) Master of Science in Nursing (MSN) from an accredited institution Minimum of 6 months of recent Nurse Practitioner experience preferred This role offers a competitive pay structure of $80-$90 per home health evaluation, along with a mileage stipend and compensation for administrative time.
