Key Responsibilities:
Perform comprehensive annual risk assessments in home settings, each averaging about 45 minutes Review prior and current medical history, conditions, and medication regimens Educate patients on their health conditions and medication usage Identify care gaps and recommend follow-up actions for comprehensive patient support Complete assessments in various locations across Shelby County, including Sidney, Fort Loramie, and Anna No requirement for developing treatment plans or prescribing medications
Qualifications:
Active Nurse Practitioner license in Ohio Board Certification through AANP or ANCC Current CPR certification (BLS) MSN from an accredited school of nursing Minimum of 6 months current NP experience preferred Enjoy compensation of $80-$90 per evaluation, with additional mileage reimbursement and paid administrative time for your efforts.
