Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage members in their homes Review and analyze patients past and current medical history, medical conditions, and medication usage Educate patients on their health conditions and medications to promote understanding and adherence Identify gaps in care and make recommendations to improve overall health outcomes Assessments onlyno prescribing medications or developing treatment plans required

Qualifications:
