Key Responsibilities:

Complete annual and post-discharge home health risk assessmentseach lasting approximately 45 minutes Evaluate patients recent and past medical histories, as well as current medical conditions and medications Educate individuals about their health conditions and medication regimens Identify gaps in care and recommend appropriate follow-up steps to resolve them Focus on assessmentno requirements for treatment plans or medication prescribing

Qualifications:

Active Nurse Practitioner License in Ohio Current CPR (BLS) certification Board Certification via AANP or ANCC At least six months of recent NP experience preferred MSN from an accredited nursing program In this impactful position, youll be compensated $80$90 per evaluation completed, with additional mileage stipend and administrative time.
