Position:
100% outpatient only Opportunity to work with a well-established team consisting of physicians, mid-level providers, licensed social workers, RNs and LPNs Close proximity to community hospital for easy access to additional services
Epic EMR No Call Minimum Qualifications:
Master of Science in Nursing NP Certification One to two years of clinical experience preferred CPR Certification Supervisory experience a plus DOT Certification is preferred
Key Responsibilities:
Conduct virtual Transition of Care visits within required post-discharge timeframes. Perform comprehensive post-hospital assessments and evaluate patients for ongoing medical needs. Complete medication reconciliation and address medication-related concerns. Review hospital records, discharge summaries, laboratory results, and diagnostic testing. Develop and implement individualized care plans. Identify and address barriers to recovery, including social determinants of health. Provide chronic disease management and primary care services when appropriate. Order laboratory tests, diagnostic studies, medications, and referrals as clinically indicated.
