  • Schedule/Shift Monday through Friday Eight-hour daytime shifts Flexible start time between 6:00 AM and 7:00 AM Typical end time between 2:00 PM and 3:00 PM No weekends or on-call responsibilities Full-time, approximately 12 weeks with possible extension Rotates among four to five Oklahoma City-area clinics Mileage between clinic locations is reimbursed Public
  • Responsibilities Provide primary care to medically complex patients receiving dialysis Complete approximately 9-10 patient visits per day Perform initial assessments and review patient medical histories Develop individualized care plans Manage chronic conditions and coordinate follow-up care Order appropriate labs, referrals, and medications Collaborate with nephrologists, clinic staff, and assigned care coordinators Educate patients about the care program and answer questions Document visits and close charts within 24 hours Public•Required Skills Active, unrestricted Oklahoma Nurse Practitioner or Physician Assistant license Primary care, family medicine, or comparable clinical experience Ability to manage medically complex and chronically ill patients Strong patient communication and rapport-building skills Comfortable working independently within a collaborative care model Willingness and ability to travel between local clinic locations Ability to complete timely and accurate clinical documentation Public•Preferred Skills Previous nephrology, dialysis, or renal-care experience Experience treating patients with chronic kidney disease Experience working with Medicare or medically complex populations Chronic disease management and care-plan development experience Familiarity with multidisciplinary or value-based care models DEA registration preferred but not required Public•Languages required N/A Public•Selling Points to Talent Predictable Monday-Friday schedule with no weekends or call Early shifts ending around 2:00-3:00 PM Manageable patient volume of approximately 9-10 visits per day Shorter visits averaging approximately 20 minutes Proprietary EHR with AI-supported clinical documentation Two dedicated care coordinators to assist with labs, referrals, and scheduling Collaborative support from nephrologists and an overseeing physician Expedited onboarding and credentialing process Mileage reimbursed for travel between clinics Opportunity to help launch and shape a growing care program Potential for extension or longer-term opportunities as the program expands
