Under supervision, the APP evaluates, diagnoses, treats, documents, and coordinates services, creating integrated treatment plans that include Behavioral Health, pharmacy, therapy, nutrition, or surgery. They monitor patient progress, refer to specialists, and participate in after-hours call rotations. The role involves practicing evidence-based care, collaborating with multidisciplinary teams, and ensuring high-quality outcomes. The APP maintains licensing, follows policies, and contributes to continuous improvement initiatives. Qualifications include a Master's in Physician Assistant Studies or Nursing, relevant licensure, certification (NCCPA or
AANC/ANNP
), and outpatient experience. Preferred candidates have 2+ years in Family Medicine, familiarity with Medical Home models, and Epic EHR experience. This full-time position offers competitive salary, benefits, and requires relocation to Prineville, OR.
