Job Description
Salary wage range:$176,393.03/yr. -$248,130.43/yr.
Job Summary:
Provide holistic care services to patients and families served by the Grand Ronde Health and Wellness Center (GRHWC). Ensure that all patients receive proper treatment in a reasonable amount of time. This includes, but not limited to: assessment, diagnosis, treatment of patients by follow-up and health promotion. Ensures that the provision of healthcare services takes place in an efficient professional environment. Contribute to accomplishment of the Tribe's holistic health and wellness objectives for clients and families.Essential Functions:
Provides professional medical screening, evaluation, diagnosis, and treatment of medical clinic patients. Assesses physical, psychological and social needs of both patient and family. Modifies plan of care with patient/family to achieve optimal health in conjunction with established treatment protocols. Recognizes office medical emergencies and coordinates treatment including facilitating EMS transfer when necessary. Orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities. Implements interventions to support the patient to regain or maintain physiologic stability. Assists with the provision of surgical care in accordance with facility, state, and federal regulations. Collaborates with multidisciplinary team members by making appropriate referrals and ensuring optimum efficiency. Facilitates staff, patient and family decision making by providing educational tools. Obtains physician consult and referral as required. Participates in after-hours call system shared by other GRHWC medical providers. Documents patient care in electronic health record effectively and efficiently. Communicates with patients by telephone and written correspondence. Discusses lifestyle changes that will promote wellbeing and health and to enhance and educate patient's participation in self-care. Participates and provides insight in health promotion activities with clinical wellness programs in the Tribal community and larger communities of Polk and Yamhill Counties. Advocates for comprehensive health care services (nutritionist, fitness specialist, childbirth educator, etc.) as needed for the health and wellness center. Collaborates with other clinical departments (lab, pharmacy, mental health and chemical dependency) for continuity of care. Attends and participates in various team meetings and assigned committees within GRHWC i.e. quality assurance, therapeutic, triage. Develops protocols for patient levels of care. Maintains certification in ACLS and PALS. Enhances current knowledge base by attending continuing education programs as deemed necessary to maintain licensure and to meet clinic goals. Performs a variety of other duties as assigned by supervisor.Education, Experience, and Licenses:
Requires Masters' of Science in Nursing with FNP focus, and requires at least two (2) years of Nurse Practitioner experience (acute care preferred); five (5) years of experience preferred. Experience working with Tribal or Public Health Clinics preferred. Licensed as Family Nurse Practitioner with prescriptive privileges in the State of Oregon and/or with the Oregon Board of Medical Examiners.Driving Requirements:
Must maintain a valid Oregon driver's license or obtain one upon hire, qualify for the Tribe's vehicle insurance, and provide proof of personal vehicle insurance.Qualifications, Registrations, and Certifications:
BLS certified. Must have some background in community health and education. Certified by a national accreditation agency. Strong knowledge base of medical conditions and treatments. Ability to perform minor office surgical procedures. Must respect all others regardless of age, gender, ethnicity, or income level. Must be committed to providing comprehensive health care to the individual, family, and community. Able to complete patient visits and electronic health record documentation in a timely manner as determined by established provider protocols. Ability to work effectively as a triage team member. Ability to work effectively in a team setting providing relationship-based care. Must be able to perform office procedures (including both invasive and non-invasive minor surgery). Well-organized and detail-oriented with the ability to establish relationships with coworkers, patients, and the public. Emotional intelligence and critical thinking skills. Effective oral and written communication skills and excellent interpersonal skills. Adherences to the organization's policies and procedures covering the privacy, access, and patient health information in compliance with Federal and state laws and the healthcare organization's information privacy practices. Must maintain patient confidentiality at all times. Knowledge of and sensitivity to Native American culture required.Environmental Factors and Conditions/Physical Requirements:
Continually requires the ability to express or receive detailed information or important instructions through oral communication. Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and/or fingers. Often requires walking or moving about to accomplish tasks. Requires standing and/or sitting for sustained periods of time, raising objects from a lower to a higher position and/or moving objects horizontally. Continually requires the use of vision, specifically close vision, color identification and depth perception and occasionally requires far vision and the ability to hear alarms. Occasionally requires lifting and carrying up to 40 pounds. This position is frequently subject to inside environmental conditions, which provide protection from weather conditions, but not necessarily from temperature changes, and is occasionally subject to outside environmental conditions. Continually required to wear plastic/latex gloves and safety shoe and is occasionally required to wear safety glasses and can be exposed to compressed gases and detergents. Continually required to handle soiled laundry. Supports a safe work environment by performing all duties in accordance with Tribal safety policies and procedures, and Emergency Management Plan. Due to the nature of interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations. May be called upon to serve as a Disaster Reservist during emergencies. Local travel is occasionally required. Travel outside of the state is infrequently required.Background Check:
