This is a value-based care role focused on quality outcomes, care coordination, and improving the health of medically complex seniors. Key Responsibilities Conduct comprehensive patient assessments Manage acute illnesses and chronic disease exacerbations Support patients during short-term rehabilitation stays Order and interpret testing, diagnostics, and interventions Ensure accurate documentation and ICD-10 coding Collaborate with interdisciplinary care teams Conduct advanced illness and advance care planning conversations Educate patients and caregivers Coordinate care transitions across healthcare settings

Required Qualifications NP:

MSN or DNP degree with board certification (AANP or ANCC)

PA:

Graduate of an accredited Physician Assistant program with NCCPA certification Active Oregon license (or ability to obtain) DEA license/prescriptive authority (or ability to obtain) Ability to obtain a collaborative practice agreement if required Valid driver's license and reliable transportation Ability to travel between facilities and perform physical assessments Preferred Qualifications 1+ years of clinical experience, preferably in long-term care Experience caring for medically complex patients Knowledge of geriatrics, chronic illness, and end-of-life care discussions Strong EMR and documentation skills Highlights Value-based care focused on quality, not volume No productivity quotas No call, weekends, or holidays Facility-based care for high-risk senior populations Strong interdisciplinary team support Excellent work-life balance and career development opportunities Travel throughout Multnomah and Clackamas Counties supporting patients in skilled nursing and senior living settings.
