This is a non-prescribing, field-based role focused on preventive care, patient education, identifying care gaps, and collaborating with primary care providers to improve health outcomes. Key Responsibilities Conduct comprehensive in-home health assessments Review medical history and medications Perform physical exams, screenings, and point-of-care testing Identify and document diagnoses for care management and treatment planning Communicate findings to primary care providers Educate members on disease management, medications, and treatment compliance Address social determinants of health and connect members with community resources Recognize and respond appropriately to urgent or emergent situations

Required Qualifications NP:

ANCC or AANP certification in

Family, Adult, Geriatric, or Adult-Gerontology Primary Care PA:
