No holidays required About the Role Optum Senior Community Care (SCC) is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care for high-risk patients in nursing homes, assisted living communities, and senior housing facilities throughout Washington and Multnomah Counties, Oregon. Providers work as part of an interdisciplinary team focused on improving outcomes for medically complex patients through value-based care. Key Responsibilities Conduct comprehensive patient assessments
Manage acute illnesses and chronic disease exacerbations
Support patients during short-term rehabilitation stays
Order and interpret testing, diagnostics, and interventions as appropriate
Ensure accurate documentation and ICD-10 coding
Collaborate closely with interdisciplinary care teams
Conduct advanced illness and advance care planning discussions
Educate patients and caregivers regarding treatment plans and care goals
