$125,000 $150,000/year $10K+ Sign-On Bonus Outpatient + In-Home Care Senior & High-Risk Patient Population Full-Time Monday Friday 8 AM 5 PM ️ 8 10 clinic patients/day 5 6 home visits/day Local travel required up to 35 miles Clinic, home-based & telehealth options ️ Malpractice Coverage 401(k) with company match + comprehensive benefits PTO, holidays & parental leave Professional development & CE opportunities Daily Responsibilities As an FNP, you ll evaluate and manage senior and high-risk patients, conduct wellness exams, review medications, perform screenings, develop individualized care plans, and coordinate follow-up care. You ll also provide patient education, support transitions after hospital discharge, collaborate with an interdisciplinary care team, and provide care in both clinic and home settings.
