Creates integrated treatment plans incorporating behavioral health, pharmacy, therapy, nutrition, and surgery; monitors patient progress and refers to specialists as needed. Works a minimum of 36 hours of patient contact weekly, participates in shared call rotation including evenings, weekends, and holidays. Collaborates with multidisciplinary teams to deliver evidence-based, personalized care, ensuring quality outcomes and patient satisfaction.
