•Geriatric Medicine for our client in Oregon! Responsibilities Provide 100% outpatient clinical care within a PACE program (Program of All-Inclusive Care for the Elderly) serving adults aged 55+, managing a daily volume of 4 to 6 patients per shift. Deliver care through a mix of in-clinic visits and off-site community/home visits to assisted living facilities, SNFs, or personal homes (typically accompanied by a team nurse). Collaborate closely with an 11-person interdisciplinary care team to coordinate patient care and management. Utilize Epic EMR for all patient management, charting, and care coordination while upholding hospital stroke-certified operational protocols. Qualifications Certified Nurse Practitioner (NP) or Physician Assistant (PA) specializing in Geriatric Medicine / Gerontology. Board-Certified or Board-Eligible status with strong/required geriatric care experience. Required life support certifications including BLS. Active Oregon state medical license.
Schedule Coverage Needed:
Active clinical coverage requested on an 11/01/2026
•Ongoing basis.
Shift Structure:
Full-time schedule covering standard business hours (Monday-Friday, 8:00 AM
•5:00 PM).
Call Requirements:
Scheduled clinical hours only with no call requirements.
Credentialing Timeframe:
60-90 days Skills Strong clinical proficiency in geriatric care, outpatient management, and PACE program workflows. Ability to conduct off-site community and home visits safely and effectively alongside care team nurses. Excellent interdisciplinary communication and collaboration skills across large care teams. Proficiency with Epic EMR systems for clinical documentation and care All travel expenses are reimbursed by our client and Orchard covers malpractice insurance with tail coverage!
