Job Description

Legacy Health is seeking a Nurse Practitioner to provide advanced primary and specialty care in an outpatient clinic setting. In this role, you will assess, diagnose, and manage acute and chronic conditions, order and interpret diagnostic tests, prescribe medications, and develop evidence-based care plans. You will collaborate closely with physicians, nurses, and interdisciplinary teams to deliver patient-centered, culturally competent care. Responsibilities include patient education, care coordination, documentation in the EHR, quality and safety initiatives, and contributing to a supportive, learning-focused clinic environment that reflects Legacy's community health mission. Responsibilities Provide advanced assessment, diagnosis, and management of acute and chronic conditions in a clinic setting Order, interpret, and act on laboratory tests, imaging, and other diagnostics Prescribe medications and treatments according to evidence-based guidelines and organizational protocols Develop, implement, and adjust patient-centered care plans in collaboration with interdisciplinary teams Educate patients and families on conditions, medications, and self-management strategies Document care thoroughly and accurately in the electronic health record (EHR) Coordinate referrals and transitions of care with community and hospital-based services Participate in quality improvement, safety, and population health initiatives Support a culture of diversity, equity, inclusion, and respectful, compassionate care Precept or mentor staff and contribute to continuous learning within the clinic Required Skills Advanced patient assessment and diagnostic reasoning Chronic disease management (e.g., diabetes, hypertension, COPD) Ordering and interpreting labs and imaging Pharmacology and safe prescribing practices Electronic health record (EHR) documentation Care coordination and referral management Patient and family education Evidence-based clinical decision-making Population health and preventive care Interdisciplinary team collaboration
