This is a non-prescribing, field-based role focused on preventive care, identifying care gaps, improving health outcomes, and collaborating with primary care providers. Key Responsibilities Conduct comprehensive in-home health assessments Review medical history and medications Perform physical exams, screenings, and point-of-care testing Identify and document diagnoses for treatment planning and care management Communicate findings to primary care providers Educate members on disease management, medications, and treatment compliance Address social determinants of health and provide referrals to community resources Recognize and respond appropriately to urgent or emergent situations

Required Qualifications NP:

ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

PA:

NCCPA certification and active state licensure Active, unrestricted license (or ability to obtain) Valid driver's license and reliable transportation Ability to perform assessments in a variety of home environments Preferred Qualifications 1+ years of clinical experience in family practice, geriatrics, primary care, or home health Experience with Medicare or geriatric populations EMR and technology proficiency Strong communication and patient education skills Ability to transport equipment up to 30 pounds and navigate stairs Highlights Preventive, non-prescribing care model In-home visits with meaningful one-on-one patient interactions Flexible schedule and strong work-life balance No call, weekends, or holidays Opportunity to improve patient outcomes through education, care gap closure, and coordination with primary care providers Eligible for student loan repayment or sign-on bonus incentives. Compensation for this specialty generally ranges from $$109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
