The Judge Group is hiring a Nurse Practitioner for a Senior Community Care (SCC) program in the Hanover, PA area. The NP will be part of a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. Schedule Mon - Fri, 8 am - 5 pm no on-call, no weekends. no holidays required Responsibilities conduct comprehensive assessments effectively manage acute and chronic medical and behavioral conditions ensure accurate and complete documentation, including ICD 10 conditions conduct advanced illness and advanced care planning conversations provide patients and caregivers with counseling and education Qualifications & Requirements active
