Role:

Medical Providers Partners PLLC is looking for a dedicated and compassionate Nurse Practitioner. This role is well-suited for early- to mid-career clinicians looking for meaningful work and a collaborative clinical environment. You will provide comprehensive medical care to approximately 20 patients per day. This position offers autonomy, consistency, and the opportunity to build strong clinical relationships. Responsibilities - Skilled Nursing Facility Provider Provide assessment, diagnosis, and management of acute and chronic conditions Conduct regular patient rounding and timely evaluations Order and interpret labs, diagnostics, and imaging Prescribe and manage medications in compliance with regulations Maintain accurate, timely documentation in the EMR Collaborate with nursing, rehab, pharmacy, hospice, and administrative teams Respond to change-in-condition and urgent care needs Support advance care planning and end-of-life discussions Participate in quality assurance and regulatory readiness efforts Qualifications Active Pennsylvania Nurse Practitioner license and required certifications Graduation from an accredited Nurse Practitioner program Strong attention to detail and timely documentation practices Self-motivated, dependable, and patient-centered Willingness to participate in a rotating on-call schedule
