As a key member of our wound care team, you will: ➢ Conduct thorough assessments and develop personalized treatment plans that address underlying health issues and co-morbidities ➢ Deliver hands-on, bedside wound care to patients in skilled nursing and assisted living facilities ➢ Lead the charge on wound assessment, treatment planning, and advanced procedures ➢ Harness cutting-edge tech and evidence-based protocols to accelerate healing ➢ Guide smart, effective decisions based on a holistic approach in collaboration with facility teams and families ➢ Keep your charting sharp and timely in our customized EMR ➢ Help drive quality initiatives and support our mission of raising the bar in post-acute care

Qualifications:

Active Pennsylvania Nurse Practitioner license (board-eligible not accepted) or Physician Assistant Prescriptive Authority and DEA license (must obtain by end of training period) Previous experience in SNFs, long-term care, rehabilitation facilities, or urgent care Willingness to travel to designated partner facilities within a 55-mile radius of home Comfortable with EMR, mobile, and laptop documentation Strong interpersonal skills — collaborative, communicative, and adaptable If you're a hands-on Provider, join us in delivering exceptional wound care where patients need it most—at the bedside.

$140,000.00 - $145,000.00 per year
