09/03/2026 Job Description Job Description Position Overview This position offers a unique opportunity to work across multiple rehabilitation environments while developing strong long-term relationships with patients recovering from injury, illness, surgery, and chronic pain conditions. The schedule is flexible and designed to support work-life balance. Practice Settings & Responsibilities Acute Rehabilitation Hospital Perform follow-up patient visits Assist with discharge planning and coordination of care Collaborate closely with rehabilitation teams and specialists Skilled Nursing Facility Provide rehabilitation-focused medical management Conduct pain management consultations Monitor patient progress and recovery plans Outpatient Office See follow-up patients discharged from acute rehab or skilled nursing facilities Provide ongoing rehabilitation and pain management care Coordinate continuity of care across treatment settings Qualifications Licensed Nurse Practitioner Strong clinical and communication skills Ability to work independently and collaboratively Experience in rehabilitation medicine preferred, but not required New graduates with strong interest in rehabilitation medicine are encouraged to apply
Compensation & Benefits Salary:
