• Nurse Practitioner (FNP) Opportunity or (PMHNP) Overview Integrated Care LLC is seeking a compassionate, highly skilled Board-Certified Nurse Practitioner (FNP) or (PMHNP) to join our growing mobile and telehealth-based practice.

This role offers a unique opportunity to provide comprehensive, patient-centered care across diverse populations, including pediatrics, adults, geriatrics, and individuals with complex medical and behavioral health needs. As a provider with Integrated Care, you will deliver care directly to patients in home settings, care homes, group homes, and via telehealth, helping reduce unnecessary hospitalizations while improving access and outcomes. This is an ideal role for a provider who values autonomy, flexibility, and meaningful impact in the community. What You'll Do Perform comprehensive assessments including physical exams, vitals, and detailed histories Diagnose and manage acute and chronic conditions in home and telehealth settings Develop and implement individualized treatment plans Perform procedures including injections, wound care, suturing, IV therapy, and phlebotomy Order and interpret labs, imaging, and diagnostic tests Provide urgent care-level services in the home to prevent ER visits Conduct telehealth visits while maintaining HIPAA compliance Document thoroughly using EMR systems (e.g., Tebra/Kareo, Epic, Cerner, eClinicalWorks) Collaborate with interdisciplinary teams including case managers, caregivers, and specialists Support chronic care management (CCM), behavioral health integration (BHI), and care coordination Provide patient and caregiver education for disease management and prevention What We're Looking For Board-Certified Family Nurse Practitioner (AANP or ANCC) Active and unrestricted state license Strong clinical experience in primary care, urgent care, or hospital-based settings Comfort managing complex, high-acuity, and underserved populations Experience with geriatric care, behavioral health, and chronic disease management preferred Proficiency with EMR/EHR systems and medical documentation Experience with procedures such as IV insertion, injections, wound care, and phlebotomy Ability to work independently in mobile and home-based environments Strong communication and clinical decision-making skills Preferred Experience (Bonus, Not Required) ICU, ER, or urgent care background Experience with home-based primary care or mobile medicine Knowledge of

CPT/ICD-10
