Job Summary We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive primary and specialized care to diverse patient populations across various settings. Your expertise will help promote health, prevent illness, and manage acute and chronic conditions with enthusiasm and professionalism. Responsibilities Conduct thorough patient assessments, including physical examinations, vital signs, and medical history reviews to determine health status. Prescribe medications, administer injections, and perform procedures such as suturing, IV infusions, and dermal fillers in accordance with state regulations and collaborative practice agreements. Manage acute and chronic illnesses across multiple specialties including geriatrics, pediatrics, behavioral health, and critical care settings. Utilize electronic health record (EHR) system like Practice Fusion for accurate documentation of patient encounters, treatment plans, and medical records while ensuring compliance with HIPAA standards. Collaborate closely with physicians, specialists, therapists, and other healthcare professionals to develop comprehensive care plans tailored to individual patient needs. Provide patient education on disease prevention, medication management, nutrition, and lifestyle modifications to promote optimal health outcomes. Participate in clinical research initiatives or quality improvement projects to advance evidence-based practices within the organization. Skills Extensive experience working with diverse populations including children, seniors in memory or assisted living care, and individuals with disabilities or complex medical needs. Proficiency in EMR/EHR systems such as Epic, Athenahealth, Cerner or eClinicalWorks; familiarity with CPT coding and ICD-10 coding for accurate billing and documentation. Strong clinical skills including venipuncture, catheterization, sterile technique, airway management, and infection control protocols. Knowledge of pharmacology for prescribing medications within DEA regulations; experience with drug administration techniques including injections and IV insertions. Ability to perform diagnostic evaluations such as sonography or laboratory specimen collection; experience in telehealth platforms is highly valued. Critical care experience in ICU or Level I/II trauma centers; emergency medicine background preferred for handling urgent cases efficiently. Excellent client communication skills to educate patients effectively while demonstrating empathy and cultural competence. Familiarity with Workers' Compensation law, hospice & palliative medicine principles, pain management strategies, and post-acute care planning enhances your ability to serve varied patient needs.
