Overview Kindred Community Cares is a new start-up multi-specialty clinic providing primary care and behavioral healthcare. We are preparing to relocate in order to expand our services. Join our dynamic healthcare team as a Family Nurse Practitioner (FNP), where you will play a vital role in delivering comprehensive, patient-centered care across diverse settings. You will support our efforts to address social determinants of health. You will utilize your advanced nursing skills to assess, diagnose, and manage a wide range of chronic health conditions, ensuring optimal outcomes for patients of all ages. This position offers an exciting opportunity to work in a collaborative fast-paced environment that values innovation, team work, and excellence in patient care. Your expertise will directly impact community health and health equity, empowering individuals and families to lead healthier lives. Duties Conduct thorough initial intake and follow-up physical examinations and patient assessments, including vital signs, medical history reviews, and physiological evaluations. Work with the patient centered team to ensure assessment of patient's levels of insecurities in regards to food, transportation, and housing needs Support development and implementation of personalized treatment plans tailored to each patient's needs, incorporating evidence-based practices. Manage acute and chronic illnesses across various specialties such as geriatrics, behavioral health, and internal medicine. Coordinate care through case management, discharge planning, and utilization review to ensure seamless patient transitions across settings like outpatient clinics, hospitals, nursing homes, and assisted living facilities. Utilize electronic health record (EHR) systems such as Athena for accurate documentation of patient encounters and medical records. Provide health coaching and education to promote lifestyle modifications, medication adherence, and disease prevention strategies. Skills Autonomous practice license is required Proven experience working with diverse populations including seniors, individuals with disabilities or developmental disabilities, and those requiring hospice or palliative care. Strong proficiency in EMR/EHR systems like Athenahealth; familiarity with CPT coding, ICD-10/ICD-9 coding standards is highly desirable. Expertise in managing complex cases involving infectious disease care, acute pain management, trauma (Level I/II Trauma Center experience), ICU or emergency medicine backgrounds preferred. Knowledge of HIPAA regulations ensuring confidentiality and security of patient information; experience with telehealth platforms is a plus. Excellent communication skills for triage assessments, patient education, interdisciplinary collaboration, and documentation accuracy. $108,341.00 - $130,475.18 per year
