Family Nurse Practitioner (FNP) - Geriatric Primary Care Eau Claire, Wisconsin Area | Full-Time | Primary Care | Geriatrics Sequoia Integrative Medical Services LLC is seeking a compassionate and motivated Family Nurse Practitioner (FNP) to join our growing team in the Eau Claire area. This is an opportunity to step outside of the traditional clinic environment and provide high-quality primary care to geriatric patients in long-term care settings . You will have the opportunity to develop meaningful relationships with your patients, practice with clinical autonomy, and work alongside a supportive team focused on improving the health and quality of life of the patients we serve. What You'll Do As an FNP with Sequoia, you will provide comprehensive primary care to geriatric patients, including: Conduct patient assessments, physical examinations, and routine follow-up visits Diagnose and manage acute and chronic conditions, including diabetes, hypertension, asthma, and other common geriatric conditions Complete admission history and physical examinations Manage ongoing patient care and respond to changes in condition Complete consult and discharge medication reconciliation Develop and manage individualized treatment plans Provide preventive care and patient education Collaborate closely with nursing staff, facility teams, and other healthcare professionals Address behavioral health needs through compassionate, whole-person care Communicate with patients, families, caregivers, and interdisciplinary teams Maintain accurate and timely clinical documentation Help promote continuity of care and positive patient outcomes What We're Looking For Master's or Doctorate degree in Nursing Current Family Nurse Practitioner (FNP) certification Active Wisconsin Nurse Practitioner license Strong clinical assessment and decision-making skills Excellent communication and interpersonal skills Ability to work independently while collaborating with a multidisciplinary team Compassionate, patient-centered approach to care Strong organizational and time-management skills Experience in primary care, geriatrics, or long-term care is preferred Why Join Sequoia?
