Our services include primary and episodic care, chronic disease management, post-discharge follow-up, medication management, health assessments, care coordination, and wound assessment and management when clinically appropriate. Services may be provided in patients' homes, assisted living and board-and-care communities, and other appropriate residential or healthcare settings. About This Opportunity CSA Nursing Care Services, Inc. is proactively recruiting Family Nurse Practitioners and Nurse Practitioners with wound-care experience and/or certification in preparation for anticipated growth in patient referrals. This is a flexible, as-needed PRN/per-diem position . Patient assignments and the start of clinical work will depend on referral volume, geographic need, completion of applicable credentialing requirements, and provider availability. No minimum number of hours, patient visits, or ongoing patient assignments is guaranteed. This opportunity may be particularly suitable for an experienced Nurse Practitioner who currently works elsewhere and is interested in flexible supplemental clinical work. Primary Responsibilities Depending on the provider's qualifications, competency, certification, and assigned patient population, responsibilities may include: Perform comprehensive and focused patient evaluations Provide mobile/home-based and appropriate facility-based patient visits Assess and manage acute and chronic medical conditions within scope of practice Perform medication review and reconciliation Order appropriate laboratory testing and diagnostic studies Develop individualized treatment and follow-up plans Provide patient and caregiver education Coordinate care with physicians, specialists, home health agencies, pharmacies, facilities, and other members of the patient's healthcare team Maintain timely, accurate, and complete clinical documentation Follow CSA policies and applicable federal, California, and payer requirements Wound Care Responsibilities Nurse Practitioners with appropriate wound-care education, experience, competency, and/or certification may also: Perform comprehensive wound assessments Evaluate pressure injuries, diabetic wounds, venous wounds, arterial wounds, skin tears, and other appropriate wound conditions Measure and document wound characteristics and healing progress Develop and update individualized wound treatment plans within the provider's scope and competency Monitor treatment response and identify potential complications Determine when additional diagnostic evaluation or specialty referral is appropriate Coordinate wound management with home health nurses, specialists, facilities, patients, and caregivers Provide wound prevention and patient/caregiver education Required Qualifications Current and unrestricted California Registered Nurse and Nurse Practitioner licenses Current Family Nurse Practitioner certification (FNP-C or FNP-BC), or other NP certification appropriate to the assigned patient population and services Current National Provider Identifier (NPI) Current BLS certification Valid driver's license and reliable transportation for mobile patient visits Strong clinical assessment, communication, and documentation skills Ability to complete required onboarding and credentialing Preferred Qualifications Currently enrolled with Medicare as a Nurse Practitioner Wound-care experience and/or recognized wound-care certification Home-based or mobile medical care experience Primary care experience Geriatric care experience Chronic disease management experience Assisted living, board-and-care, SNF, or post-acute experience Experience caring for medically complex patients DEA registration when applicable to assigned clinical responsibilities Service Areas CSA Nursing Care Services, Inc.

Patient assignments will be offered based on patient location, referral volume, provider availability, qualifications, credentialing status, and reasonable travel distance. Schedule PRN / Per Diem — Flexible There is no guaranteed minimum number of hours, visits, or patients. Providers will communicate their availability to CSA. Patient visits will be offered based on referral volume, geographic needs, provider availability, qualifications, and applicable credentialing requirements. Credentialing Selected candidates must complete CSA Nursing Care Services, Inc.'s onboarding and credentialing requirements before providing patient services. Patient assignments involving specific insurance plans may depend upon completion of applicable payer enrollment, credentialing, contracting, or reassignment requirements. Compensation Competitive per-diem compensation based on qualifications, clinical experience, assigned responsibilities, and applicable compensation requirements. Compensation details will be discussed during the interview process.
