Manage a mature panel of 75-80 seniors with a gradual ramp-up.Work-Life Balance:
Predictable Monday-Friday schedule with a dedicated weekly remote administrative day . Mon-Wed (On-Site): Clinic visits, acute assessments, daily IDT meetings. Thu (Community): Local nursing facility / SNF / home visits. Fri (Remote): Work-from-home charting, care coordination, follow-ups.Light On-Call:
Dedicated Nurse Triage handles first-line calls; provider escalation is rare and receives supplemental call pay .Autonomy & Support:
Direct reporting to the Medical Director with zero RVU quotas. Requirements Active (or eligible for expedited) Colorado NP or PA license & DEA certification. AGPCNP, FNP, ACNP, or NCCPA certification. 2+ years experience in geriatrics, internal medicine, family practice, SNF, or home care preferred (standout new grads with strong RN/rotation background considered) .
