As a key member of our clinical team, you will deliver high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home. Responsibilities:
Provide high-quality preventative care to residents in skilled nursing facilities, utilizing remote patient monitoring, chronic care management, and collaborative care management approaches Conduct on-site visits and work closely with a dedicated scribe team for efficient documentation Build strong relationships with facilities and maintain a patient-centered approach Collaborate with other team members to ensure seamless care coordination Participate in quality improvement initiatives and contribute to enhancing patient outcomes
Requirements:
Active NP or PA license in the state of practice 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Strong problem-solving and critical thinking abilities
