Nurse Practitioner (NP) - PART_TIME Carease Health - MI is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. About the

As a Nurse Practitioner (NP) at Carease Health - MI , you will have the opportunity to make a real difference in patient care while working in a dynamic and supportive clinical environment.
Your responsibilities will include:
This is a part time position of 18 hours per week. Providing high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work Building strong relationships with the facility staff and residents Maintaining a patient-centered approach and being a collaborative team player Documenting patient care using a dedicated scribe team
Requirements:
Active NP or PA license (in the state of practice) 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills
