We're a Georgia-based mobile healthcare organization partnering with physician practices, assisted living facilities, and long-term care settings to deliver Primary Care, IV Infusion Therapy, Advanced Wound Care, Rounding Visits, and more — combining clinical excellence with the convenience and compassion of care delivered in familiar surroundings. We're looking for a compassionate, skilled Nurse Practitioner to join our mobile care team. In this role, you'll deliver a broad scope of services — Primary Care, IV Infusion Therapy, Advanced Wound Care, Facility Rounding Visits, and Telehealth — to patients in their homes, assisted living facilities, and long-term care settings. You'll assess, treat, and monitor patients across acute and chronic conditions, promoting healing, preventing complications, and improving quality of life, all while working independently in the field and staying closely connected to patients, families, and the broader care team. What You'll Do
- Conduct primary care visits, chronic disease management, and preventive care & wellness screenings
- Administer IV infusion therapy, including hydration, antibiotics, and immune support
- Conduct advanced wound care evaluations and develop individualized treatment plans
- Perform sharp and non-sharp debridements, apply skin substitutes and grafts, and administer PRP therapy
- Assess and manage lymphedema
- Conduct rounding visits at facilities (ALF, SNF, Rehab, LTCF), including post-hospital follow-up and medication review
- Deliver evidence-based care via telehealth where appropriate
- Coordinate care, order labs & diagnostics, and complete preoperative medical evaluations
- Educate patients and caregivers on treatment plans and prevention
- Maintain thorough documentation and communicate proactively with referring providers and care teams
- Ensure compliance with WavLinQ's clinical and regulatory standards What We're Looking For
- Active, unrestricted Nurse Practitioner license in Georgia (FNP, AGPCNP, or equivalent)
- Board Certified (ANCC or AANP)
- Experience across one or more of the following preferred: primary care, chronic disease management, IV infusion therapy, or wound care
- Certification in Wound Care (WCC, CWCN, or equivalent) a plus for wound care caseload
- IV therapy certification/competency a plus for infusion caseload
- Strong clinical assessment and decision-making skills across a broad scope of practice
- Excellent documentation and communication skills
- Valid driver's license, reliable transportation, and willingness to travel throughout assigned service areas
- For the Virtual role: active NP license in Georgia and ability to deliver evidence-based care via telehealth Compensation WavLinQ NPs are engaged as independent contractors (1099), paid per completed, billable visit — giving you control over your schedule and the opportunity to grow your earnings across multiple service lines, from primary care and IV infusion therapy to advanced wound care and facility rounding.
