Nurse Practitioner Job Summary:

Carease Health - WI, a nationwide preventative healthcare company focused on reducing hospitalizations, is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner at Carease Health - WI, you will be part of a hybrid role that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

Provide high-quality preventative care to residents in skilled nursing facilities Conduct on-site visits and remote work from home, supported by a dedicated scribe team Build strong relationships with facility staff and maintain a patient-centered approach Collaborate with other healthcare professionals to ensure seamless patient care Maintain accurate and timely documentation of patient visits and treatment plans

Requirements:

Active NP or PA license (in the state of practice) 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Masters-level education in Nursing or a related field About the
