Nurse Practitioner Job Summary:
We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Carease Health. As a Nurse Practitioner with our organization, you will have the opportunity to deliver high-quality preventative care to residents in skilled nursing facilities, while enjoying a hybrid work arrangement that combines on-site visits with remote work from home. Our team is dedicated to improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care.Responsibilities:
Provide preventative care to residents in skilled nursing facilities, including conducting on-site visits and remote monitoring of vital signs Collaborate with healthcare teams to develop and implement individualized care plans, while ensuring continuity of care and coordination with other healthcare providers Conduct documentation and charting in a timely and accurate manner, with the support of a dedicated scribe team Participate in multidisciplinary team meetings and clinical decision-making, utilizing your expertise in Geriatrics and Internal Medicine Build strong relationships with facility staff, patients, and families, while maintaining a patient-centered approach Stay current with industry developments and best practices through ongoing education and trainingRequirements:
Active NP or PA license in the state of practice 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Ability to work independently and as part of a team Strong problem-solving and critical thinking skillsBenefits:
