Nurse Practitioner Carease Health - IL Carlyle, Illinois As a Nurse Practitioner with Carease Health - IL, you will play a crucial role in delivering high-quality preventative care to residents in skilled nursing facilities. Our company prioritizes proactive measures for enhanced physical health and well-being, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program.
Responsibilities:
Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home Conduct assessments, develop treatment plans, and prescribe medications as needed Collaborate with healthcare teams to ensure comprehensive care and minimize hospitalizations Utilize RPM, CCM, and CoCM to enhance patient outcomes Maintain accurate and timely documentation of patient care and outcomes Participate in quality improvement initiatives to enhance patient care standards
Requirements:
Active NP license (in the state of practice) 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills
