This vital role offers the opportunity to deliver expert wound management and advanced patient care across diverse settings. You will collaborate with a multidisciplinary team to develop and implement individualized treatment plans, utilizing cutting-edge techniques and technologies. Your expertise will directly improve patient outcomes, promote healing, and enhance quality of life for individuals with complex wounds. Duties Conduct comprehensive patient assessments, including physical examinations, vital signs, and detailed wound evaluations. Develop, implement, and monitor personalized wound care plans utilizing advanced techniques such as debridement, dressing selection, and skin grafting when appropriate. Administer specialized treatments including Botulinum toxin injections for specific conditions and perform suturing for wound closure. Manage complex cases involving diabetic ulcers, pressure injuries, venous stasis ulcers, and other chronic wounds through evidence-based practices. Collaborate closely with healthcare professionals across disciplines—such as infectious disease specialists, dermatologists, and physical therapists—to optimize patient outcomes. Utilize electronic health record (EHR) systems like Epic or Cerner for accurate documentation, order entry, and data management. Educate patients and caregivers on wound prevention, care techniques, and lifestyle modifications to support healing and prevent recurrence. Experience Proven clinical experience in wound care management within hospital settings, outpatient clinics, or long-term care facilities.(not required) Demonstrated proficiency with EMR/EHR systems such as Gerimed for medical documentation and patient tracking. Knowledge of CPT coding and ICD coding standards for accurate billing and documentation processes. Background working with diverse populations including pediatrics, geriatrics, hospice & palliative medicine, and individuals with disabilities or complex medical needs.
