We are seeking a dedicated Psychiatric Mental Health Nurse Practicitioner to join our team and provide exceptional mental health care to patients in a skilled nursing facility. Scope of job:

To complete onsite patient initial evals, follow-up assessments, and rounding's 2 days per week in a skilled nursing facility.

Duties:

Conduct onsite initial evaluations, follow up assessments, and discharge for patients in a skilled nursing facility Rounding on patients Develop personalized treatment plans incorporating psychotherapy and other behavioral health interventions. Manage medication regimens responsibly while monitoring patient progress and side effects in compliance with HIPAA regulations and medical documentation standards. Develop personalized treatment plans for patients Prescribe and monitor medication regimens Maintain accurate medical records, document intake assessments, treatment progress notes, and discharge summaries following best practices in medical terminology and data collection.

Required:

Master's degree in nursing with a specialization in Psychiatric Mental Health Nurse Practitioner from an accredited program. Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) with current VA state licensure.
