ASAP Job Requirements Graduation from an accredited health professions school with active national board certification as a Nurse Practitioner or Physician Assistant Active licensure in the state of Rhode Island Active DEA licensure and state-specific controlled substance registration, as applicable Certification in Basic Life Support (BLS) Ability to travel as needed with proof of liability and property damage insurance on vehicle used Minimum of 1 year of experience in a relevant healthcare field Responsibilities Deliver clinical care both in-person at patients homes Provide compassionate, individualized, and holistic patient care, including comprehensive primary care, urgent care, and chronic condition management Collaborate with a multidisciplinary care team for transitions of care support and complex care management Engage in professional development opportunities and collaborate with experts in geriatric medicine and palliative care Participate in after-hours on-call coverage as needed to join our healthcare team and make a difference in the lives of our patients.
