Nurse Practitioner LiveWell Homecare Agency Columbia, SC Job Details Full-time $50 - $60 an hour 6 hours ago Benefits 401(k) Qualifications RN License Acute care facility experience Nurse Practitioner (NP) Advanced practice provider Full Job Description We are seeking a dedicated part-time Nurse Practitioner to provide direct patient care in Skilled Nursing, Assisted Living, and Rehab facilities, working closely with an MD. Deliver personalized post-acute and long-term care following treatment plans and facility protocols. Evaluate new admissions, conduct follow-ups, and manage acute cases daily. Coordinate with primary care, specialists, families, and facility staff to ensure seamless care and compliance. Review lab and radiology reports, document in EMR, and handle billing as per guidelines. Participate in quality improvement initiatives and represent PSG professionally. Maintain appropriate census and assist with discharge planning. Qualifications include ANCC certification, SC license preferred, RN certification, and experience in acute care. Benefits include learning opportunities, career advancement, and 401K. Work hours are Monday to Friday, 8 am-4 pm, three days per week. Compensation ranges from $50 to $60 per hour.
