Job Description
We are seeking a dedicated and experienced Nurse Practitioner or Physician Assistant to join our Family Medicine team as a floating provider. This role supports outpatient care across multiple community clinics in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties, delivering patient-centered primary care with an emphasis on prevention and chronic disease management. Responsibilities Provide comprehensive primary care to patients across the lifespan, including wellness, acute care, and chronic condition management. Travel regularly among assigned family medicine sites within Abbeville, Greenwood, Laurens, McCormick, and Saluda, SC. Deliver evidence-based care and partner with patients and families to develop and explain treatment and health maintenance plans. Collaborate with physicians, nurses, and care teams to coordinate patient care and referrals. Accurately and promptly document patient encounters in the electronic health record. Minimum 2 years' experience in primary care or family medicine. Active, unrestricted South Carolina medical license. Certification or eligibility for certification in specialty (FNP or PA-C).
