Job Description
We are hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to serve as a floating primary care provider across outpatient family medicine sites in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties, SC. Responsibilities Provide comprehensive primary care for patients across the lifespan: wellness, prevention, acute care, and chronic disease management. Travel regularly to assigned family medicine clinics within the five-county region; reliably cover scheduled clinic shifts. Perform patient assessments, develop and implement treatment plans, prescribe and manage medications per scope of practice. Coordinate care with physicians, nurses, and ancillary staff; facilitate referrals and follow-up. Educate patients and families on diagnoses, treatments, medications, and preventive health. Accurately document encounters in the electronic health record in a timely manner and complete required administrative tasks. Minimum 2 years' experience in primary care or family medicine. Active, unrestricted South Carolina license (or eligible). National certification (FNP, PA-C) or eligibility for certification. Valid driver's license and reliable transportation for inter-site travel. Strong clinical judgment, communication skills, and ability to work independently in varied clinic settings.
