Job Description

Job Summary Sandhills Medical Foundation, Inc. is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) . As a Federally Qualified Health Center (FQHC) serving the community since 1977, our mission is to deliver quality, comprehensive, and cost-effective healthcare to responsive community needs. In this role, you will function as an essential member of a Patient-Centered Medical Home, providing primary care to patients across the lifespan. Key Responsibilities Diagnose and treat acute illnesses, injuries, and manage chronic conditions. Provide comprehensive primary healthcare and wellness exams within established guidelines. Order and interpret laboratory studies and diagnostic testing. Prescribe therapeutic treatment plans and medications. Coordinate care with specialists, testing facilities, and community health teams. Maintain accurate, HIPAA-compliant patient charts within the Electronic Medical Record (EMR) system.
Qualifications Education:
Master's or Doctoral degree in Nursing (MSN/DNP) from an accredited program.
Licensure:
Board certified Family Nurse Practitioner (FNP) with an active, unrestricted Nurse Practitioner license.
Experience:
1-2 years of experience in family practice, internal medicine, or community health environments is preferred.
Skills:
