Nurse Practitioner Job Overview PrePost+ is a group Pre-Operative Clearance and Post-Surgical Primary Care Centers which directly addresses the growing primary care physician (PCP) shortage while aligning with the CMS Transforming Episode Accountability Model (TEAM). By offering dedicated post-surgical follow-up care, integrated rehabilitation, and expedited pre-operative clearances, the center fills critical gaps caused by limited PCP availability, reducing surgical delays and rehospitalizations. It offloads the burden from traditional PCPs by employing nurse practitioners and physician assistants, and expands access through telemedicine. Strategic partnerships with hospitals, surgical centers, and insurers ensure patient continuity and access. Financially, the model is sustainable through diversified revenue from clearances, post-op care, and rehab services, making it a scalable, TEAM-aligned solution to improve outcomes and control costs. Responsibilities This is a full-time, on-site role for a Nurse Practitioner/Physician Assistant (NP/PA), based primarily out of our Clarksville, TN location, with occasional travel to our Nashville or Springfield locations as needed. The NP/PA will be responsible for providing primary care services, conducting physical exams, diagnosing and treating illnesses, prescribing medications, and managing patient health records. The role also includes coordinating with other healthcare professionals, educating patients on managing their health, and ensuring high-quality patient care. The NP/PA is expected to adhere to all medical regulations and guidelines and contribute to the continuous improvement of patient care practices. Qualifications Physical Examination, Diagnosis, and Treatment skills Experience in prescribing medications and managing patient health records Ability to coordinate with other healthcare professionals and provide patient education Knowledge of medical regulations and guidelines Excellent communication and interpersonal skills Master's degree in Nursing or Physician Assistant Studies Valid state licensure as an NP or PA Experience in primary care or family medicine is preferred Ability to work independently and collaboratively in a healthcare setting Surgery and or Cardiology Experience a plus
