Job Summary We are seeking a dynamic and compassionate Nurse Practitioner to join our healthcare team, delivering high-quality patient care across diverse medical settings. In this role, you will serve as an advanced practice provider, collaborating closely with physicians and healthcare professionals to assess, diagnose, and manage patient health. Your expertise will support a wide range of clinical services, including acute and chronic care management, preventive health, and specialized treatments. Duties Conduct comprehensive physical examinations and patient assessments to determine health status and develop personalized care plans. Prescribe medications and treatments in accordance with state regulations and collaborative practice agreements, ensuring safe medication administration and adherence to HIPAA guidelines. Manage acute and chronic conditions across various specialties such as internal medicine, geriatrics, pediatrics, and behavioral health. Perform procedures including suturing, IV infusion, and sterile processing techniques. Utilize electronic health record (EHR) systems using Athena One for accurate medical documentation, specimen collection/processing, and medical records management. Collaborate effectively with healthcare professionals across disciplines to coordinate patient care, and utilization review. Provide telehealth consultations to extend access to care remotely while maintaining high standards of clinical evaluation and patient communication. Qualifications Valid Nurse Practitioner licensure with current certification in advanced practice nursing. Demonstrated experience working with diverse patient populations including pediatrics, geriatrics, hospice, or individuals with developmental disabilities. Proficiency in EMR/EHR system using Athenahealth; experience with telehealth platforms is preferred. Strong knowledge of physiology, medical terminology, CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-10/ICD-9 classifications for accurate diagnosis coding. Skilled in performing clinical procedures including venipuncture, phlebotomy, sterile techniques for injections or suturing; experience with sonography or medical imaging is a plus. Knowledge of Workers' Compensation law and hospice care practices; experience working within multidisciplinary teams is essential. Excellent client communication skills with the ability to educate patients on treatment plans and health coaching strategies.
