As a Whole Person Care Nurse Practitioner , you'll provide primary care services to patients of all ages while collaborating with an interdisciplinary team to deliver integrated, patient-centered care that improves health outcomes. This position requires traveling to the following clinics: Angleton Alvin Lake Jackson Clute Freeport Your Responsibilities Assess, diagnose, and manage the healthcare needs of patients across the lifespan. Obtain comprehensive health histories through patient interviews, family discussions, and physical examinations. Identify health risks and develop appropriate treatment plans based on clinical findings, diagnostic testing, and patient assessments. Provide wellness assessments, preventive care, and chronic disease management. Counsel and educate patients on health conditions, treatment plans, and health-promoting activities. Collaborate with physicians and other healthcare professionals to deliver coordinated, whole-person care. Refer patients with unstable or complex conditions to specialty care when appropriate. Maintain complete, timely, and accurate patient documentation. Ensure prompt follow-up regarding laboratory results, referrals, medication needs, and patient inquiries. Support quality improvement initiatives and contribute to positive patient health outcomes. Maintain professional licensure and continuing education requirements. Perform other duties as assigned.
