Job Description

Nurse Practitioner - Transitional Care and Wound Care MidSouth is seeking a full-time Nurse Practitioner to provide in-home medical visits and telemedicine services to patients in West Tyler, Texas, and the surrounding communities. Applicants must live within or near the designated service area. We specialize in transitional care and wound care services. Our team helps patients safely transition home following hospitalization while providing ongoing medical management and specialized wound care in the home. Nurse Practitioners work closely with home health agencies, primary care providers, specialists, and other members of the patient's care team. Responsibilities include evaluating and treating patients in their homes, managing transitional care needs, assessing and treating acute and chronic wounds, developing wound care treatment plans, and providing follow-up care through in-person and telemedicine visits.
